Group & outline: dots instead of plus & minus button

Howdy,
I have 14,000 records in 8 levels. No matter which order I group in (starting from level 1 or level 8), some of the groups show the plus/minus signs, but some show only a vertical column of dots that indicate which level the group's in.

Sometimes this happens within the same level -- i.e., I group one set of records on, say, level 3 and get the plus/minus, then immediately group another set and get the dots.

Clicking on the level numbers collapses and expands everything properly, but without the plus/minus I can't expand selectively.

Thanks for any suggestions.
