How to create form where the data will be used within the same spreadsheet

I have searched the forums but can not find an answer specific to my needs. Here is the scenario:

We use a datasheet where we input data in different places on the sheet, such as employee name, employee number,etc., along with other data. The form is a word doc. It is a pain to click around to the different places on the form and input the data. The doc file is a mess.

My idea is - re-create the form in Excel. Each place where data has to be typed will be it's own cell. Then, create a form that is attached to the spreadsheet. The form would be simple. A box that reads "enter name here", a box below that that reads 'enter employee number here" etc. Then, the cells in the spreadsheet will automatically duplicate what is typed in boxes on the form into the appropriate cells in the spreadsheet.

Last - I would like a button on the form that saves the completed spreadsheet using a specific file naming criteria, then have the form reset itself, open a new blank master spreadsheet that is ready for the next person's data.

Any help would be appreciated!
