Macro startup user entry

I'm writing a macro to transfer data from one worksheet to another worksheet based on four search criteria. On initial startup of the macro, is it possible to display a small form (like the message box) where the user is prompted to enter two integers and two dates.
 

Excel Facts

Remove leading & trailing spaces
Save as CSV to remove all leading and trailing spaces. It is faster than using TRIM().
look to create a userform in the VBA screen then add the 4 textbox with labels and then set the command when the workbook opens it opens the form. Add a command button to the form to function what ever it is you want to happen when they have completed the form text fields.
