Making respondents rank options in a order for a survey

I am creating a survey form in excel and wanted to know how can I make the respondents rank a few options in a particular order of their choice. I typically would want there to be 2 boxes visible to the respondent, one of them containing the options and the other one their order of preference. also the required buttons for adding the info on the second box.
Kindly help me out.
