Pull a column from a different sheet on or after a certain date and convert that into a number by counting a certain word

I would like to pull a response from a column (O2:O) from sheet "Form" and the date info for that response is in "Form" B2:B. The sheet and cell it will be going to is called "A2" in cell K2 and there will be a date in J2 that will need to be the date that it starts pulling info from the other sheet(but not any blank responses). The response from Form will also have responses like Trial 1 Trial 2 Trial 3 and when it pulls to K cell I would like the formula to divide by five to give me a number that I can convert to a percentage
