Returning a value from one table on a sheet to a table on another sheet

Hi all - been wracking my brains over this during the last couple of days - but am stuck.

I have a table on a Sheet called "Week By Week" - that details teams - with a score and a ranking position.

I have a table setup on another sheet called "Week By Week Table" - that summarises several items.

What I want to do on the Week By Week Table sheet is to look up the Team name from the column. the Date from the row - and then return the value in the "Position" column from the source data.
