SPC Chart ... I think ..?????

Hi I've tried to paste a .jpg without success.. Has anybody seen an SPC chart which appears to be displayed in one cell it has 5 colours 2 green, the middle light blue, then amber and red.

It also has a diamond/star representing our performance and a vertical bar line representing the national average.

Not sure how clear this is ... I can only live in hope....
