Take Macro output and createa

I have this workbook where I enter information from external sources into the input tab then I manipulate it through Macros. On the Conference History tab, I have the output of each unique value from every row in the Input tab with the corresponding columns (years) that each unique value that appears in the row. Now I want to take the Conference History tab and create a macro where I create a list on a separate sheet of each unique entry with the listing of each school and the years it participates in each unique entry.

Here is a view of what I am looking to do. I take this output

Virginia01ACC2020-2024
Virginia Tech01ACC2020-2024
Wake Forest01ACC2020-2024
Central Florida01BIG12C2024-202401AMAC2020-2023
On the new sheet it would look like

01ACC
Virginia 2020-2024
Virginia Tech 2020-2024

Central Florida 2024-2024

01AMAC
Central Florida 2020-2023

and so on
