I am trying to build a VBA, but it is not working well for me. Please assist.

Here is my situation:
I would like a box to poo out when a client select yes to a question. The data name is Any Criminal History (Yes/No). If Yes, a comment box will pop out so client could enter where, when, and what.

please assist.
