Vlookup or Index(match) with concatenating values

in both the lookup value and in the array, I need to concenate two values to avoid pulling the wrong number. what is the best way to do this?

for example: say sheet 1, where I am doing the vlookup (column C)
for the lookup value I want to concatenate same row, columns A and B.
then in Sheet 2, my table array would would start in column C to D - with C having Also, A and B concatenated. With pulling the result in D.

I have tried this multiple ways and I keep getting #N/A
