Change name of Tabs using a macro

I want to change tab names based on text entered in cells in a row. The individual tabs are hyperlinked to the cells. The Tabs are named as Item #1, Item #2 etc
I require Item #1 tab to be changed to "VMS Roller/Thermal Rolls Reception" and so on.
Sample data is as under:
Item #1 VMS Roller/Thermal Rolls Reception
Item #2 DETTOL SOAP
Item #3 Gate Pass - Non-Returnable
Item #4 Gate Pass - Returnable

It would be easier for me to create the hyperlinks afresh than entering the names of the sheet tabs if it is possible to have the name changed for the sheet tabs.
