Checkboxes

I'm doing a form with 25 questios where the score is from 1 to 5, I'd like to setup 5 checkboxes in such a way that only one out of the five could be ticked per each question, like such:

e.g. How do you rate today's soup? 1 2 3 4 5

Any ideas?
