Count by Month

I have a set of data with dates in Col A and ranks in Col H.

How do you count how many entries/rows are for each month (Col A)?
How do you count how many times column H says "3" for each month and how many times it says "2"?

I know this is possible using a pivot table, but how would you accomplish this with basic formula?
