Create submission database/table using cells copied from worksheet

Hello, I have created a form on worksheet 1 which has 10 specific cells that I would like the values from those cells to be populated into a table into another worksheet. This form leverages a number of drop down list options and xcopy entries from a table that sits in worksheet 1. Each time the form is filled out, I'd like to submit specific cell entries to a second worksheet. Each form submission would append after the previous.
