Fill in a PDF Form from Excel

I have an excel file that contains all the information and would like to import the proper info to the proper spots on this PDF form. Can this be done?

I attempted to make an Excel sheet that looked like this form but was struggling with it...

Excel Facts

Highlight Duplicates
Home, Conditional Formatting, Highlight Cells, Duplicate records, OK to add pink formatting to any duplicates in selected range.
I suggest creating the form in Word and doing Mail Merge to populate the form with data from Excel.
