Fill Out Forms to Copy and Paste

Hi,

I want to be able to provide people with a template which gives examples or a description, which they can fill out and copy - to be able to paste elsewhere.
But the template that copies only includes the item and the answer (not the description)

Eg.

Name: (Your Full Name)
Address: (Your full address and postcode)
DOB:
Favorite Chocolate Bar:
Pets: (Give details of their name and breed)

When this is filled out I want them to be able to see the descriptions but when they go to copy it - they do not copy.

Name: Michelle (Your Full Name)
Address: 1 The Street, L15 4BP (Your full address and postcode)
DOB: 01.01.1990
Favorite Chocolate Bar: Mars
Pets: 1 Dog, Pug called Charlie (Give details of their name and breed)

Name: Michelle
Address: 1 The Street, L15 4BP
DOB: 01.01.1990
Favorite Chocolate Bar: Mars
Pets: 1 Dog, Pug called Charlie

Maybe this could be a form which when filled out appears somewhere else without the descriptions to be able to be copied????
