FORMS - Pulling in Data based on a value entered

I have a register that contains information on people each row has a unique number and what I have done so far is created a form where we can input data and it will add the data to the raw data sheet in certain cells.

What I would like to do as well is view certain information by person, this would be done by adding a form field where the name of the person would be entered and it would either count the number of Issue Types (There are 3 issue types that can be entered into the register)
or display a combination of data based on other drop downs.

So If I wanted to view details let's say on Bob and I want to see how many issue types he has and other information that is corralating to that issue, I could view this in a graph or some sort of pivot chart/table.

Any ideas on how to get this going would be great!!!
