INDEX MATCH over multiple columns

I am trying (On Sheet6) to put a formula of INDEX MATCH

Excel Formula:
=INDEX('Sheet_4'!A:A,MATCH('Sheet_6'!G2,'Sheet_4'!C:C,0),MATCH('Sheet_6'!D2,'Sheet_4'!E:E,0))

So, I am trying to return Column A from Sheet 4.

D2 looks up Sheet 4, Column E
G2 looks up Sheet 4, Column C

Can anyone help, and advise where I am going wrong please :)
