lookup values from 5 different worksheet HELP

hi all,

I have gone through tone of post regarding the lookup issues from different sheets, most the feedback advising to install "the morefunc.xll add-in", whenever I tried and tried, the sites not exist. Could someone have another suggestion or have another function to solve this issue?
