Problem with Radio Button Form controls

I asked my students to create a 6 radio button form control. With I about 20% of the students there was an issue. The completed form allows the user two select two radio button. It also makes the cell link report an irratic result. If I go in to uncheck one of the buttons on the Format Control - Control tab, it does not fix the problem. Can someone tell me what the students are doing to cause this to happen and more importantly, how do you fix it?

I have attached an image showing the problem.

Apparently they created a "Group Box" and put the option buttons inside, but the option buttons below (20, 25 and 30) are not totally inside.

All they have to do is insert the option buttons into the Group Box:

All they have to do is insert the option buttons into the Group Box:
