Consolidate road data (from and to)

I am looking for a way to automate consolidating project numbers and making descriptions match. Here is an example:

I have a very long road segment list (800+ segments) with the structure shown in the table below, which I want to consolidate into one row in a new tab with these columns: project, road, from, and to (in this example, 556, Main St, Anclote, Moog Rd).

How do I tell excel to give me the correct from and to for a project in my consolidated list? They aren't always in this order with Anclote in the top left and Moog in the bottom right.
