Creating a titled sheet from a list, then and apply a template

Hi There,

Any help with the following query would be greatly appreciated.

I have an excel sheet titled "Main List" which contains a list of company names in column D. I’m looking for a macro (to be applied to a button) which creates a sheet and titles it according to the company name selected from Main List. What’s more is that I also need the newly created sheet to have a template applied to it.
