Dynamic External Hyperlink

Hello!

I am trying to create an automatic function where as an external website hyperlink is created based on the cell data within my document. I currently have column D setup so that when columns A-C are updated, it generates the value that needs to be replaced in each external hyperlink. I am looking for a way for the bold section of the external hyperlink in Column E to be replaced with the value of Column D.

I have attached a snippet with an example of what I'm trying to achieve.

ABCDE
12123​
100​
12345​
12123-100-12345<-I would like to automaticlly generate hyperlink that uses Cell D2 to replace the BOLD number in the example URL below:
12323-100-00001/EN/lastreleased/viewable/
 

Excel Facts

Format cells as time
Select range and press Ctrl+Shift+2 to format cells as time. (Shift 2 is the @ sign).
