Form Single Time Line with Multiple Date Fields

I'm working in Excel 2013 for windows. I 'm wondering if there is any way that I can take a table that tracks 4 different types of events for each record(row). What is the best method to get all of the events for all records placed into one chart where the horizontal axis is a timeline.

Example Dataset:

Customize Quick Access Toolbar. From All Commands, add Speak Cells or Speak Cells on Enter to QAT. Select cells. Press Speak Cells.
Create a contiguous time line across the top of your sheet, starting with your earliest date, ending with oldest date.
Items 1 through 4 remain as you have them in your example.

enter the assignment numbers in their corresponding cells.

i.e, Assignment 1 will be entered under date 08/15/2015 for Item 1.
