Formula for worksheet needed

I am a nurse administering vaccinations for the commonwealth of Massachusetts. I have one book with several sheets, one sheet for each formulation of flu and one sheet that’s keeping track of my totals. I’d like to create a sheet that will be a distribution log which I’ll give to the people picking up. So the formula needs to say—If you see this Site number (Identifier) and this date Copy this Block of info (which will be Lot number Expiration and number of doses From each of the sheets and sections, onto a printable hand out. Got any ideas?
