Pivot Table, stack Row Label?

My Pivot Table must be condensed to fit a printed page. How can I stack the fields to achieve a look like this:

I've tried FieldSettings compact form but it doesn't line up properly. I'm using Excel 2007.

Thanks for any help you can offer!
 

Excel Facts

What does custom number format of ;;; mean?
Three semi-colons will hide the value in the cell. Although most people use white font instead.

Creative thinking suggestion - in your data, concatenate the email and phone field so it is one field instead of two.
