Unique values in Range & sum of unique values

In columns BQ,BT I'm using Index/Match to list Unique Values. These values determine hourly rates for regular hours (Columns BR,BU) and overtime hours(Columns BS,BV). How can I use excel to list the Unique Values in Column (BY) and the sum of regular hours for each value in (BZ) along with the sum of each values overtime hours in (CA)? The screen shot below is an example...I've manually entered in what the desired output would be with red font.

I do not know how to copy a screen shot into an excel sheet and work on it

in excel sheet as far as possible do not have blank rows, blank column or blank cells

redesign the sheet data and just select these data and copy into the message area .
if you are conversant with the addin in this newsgroup it is possible to transfer the sheet data.

another method is to use advanced filter to get unique data provided excel sheet follows the conditions given above.
