Insert a column between C & D to concatenate columns A, B, and C, like: A13 &"|" & B13 & "|" & C13
(you can only use one field for the next step ... and the "|" separators will allow you to split the field later on in the process)
Now use the range from the new column D12 downwards and rightwards for your source data table for normalizing your data this way:

When you have your final list, you can split your joined field using Data/Text To Columns ... either in a new area, or making space for the 1 to 3 column transformation.
