Convert source TXT to a properly formatted CSV

Greetings -
I am attempting to convert a source TXT file to a destination CSV and have it properly formatted as it is in the destination CSV below. If you open the "Destination" text below as a csv, you can see the desired CSV format. I have an .EXE which accomplishes this in Windows XP x86, but does not work in Windows 7 x64. The problem is that we do not have the source macro that was used in compiling the exe. I was hoping that someone on here can help because we cannot figure out how this conversion was accomplished. This is a similar request to this post http://www.mrexcel.com/forum/excel-questions/509264-macro-convert-txt-csv-files.html but the information provided in that post does not solve our issue.
We would prefer this to work with Office 2010, but any recent Office version (2010 or 2013) will do.
I would highly appreciate some assistance on this issue. Thank you in advance.
