Formula or Macro to sort data from data.

Hi I am looking to try and convert data that have been collected off some survey forms. The data appears like this below. I am wanting to create lists like shown further down. Is there a formula that i can use to copy down? I am looking at trying to convert around 200 rows into this list shown at the bottom of the page.

Could you please assist me
