Pulling info from a table based off store number

Hello, I am struggling with this formula. I was using index function, maybe I shouldn't be. I have an online form that people can put their store numbers in and submit commissary issues. I want to keep that form short. So on the back end, I want to fill in the City, State etc. columns by matching their store number input. On the right is a table with store numbers and their appropriate City, State, DMA, etc.

B2 is entered in the webform and goes to my database. I want the appropriate city, state, DMA, and franchisee to be pulled from M-Q to fill it's appropriate cell in H-K.

Excel Facts

Show numbers in thousands?
Use a custom number format of #,##0,K. Each comma after the final 0 will divide the displayed number by another thousand
Try to post directly here in an Excel readable a small sample of relevant data along with the output that you require.
