Changing a formula

I have "inherited" a workbook from a past worker and there is a formula that doesn't give the information required. The formula posted below works but I needs extra criteria.

I want it to match against 3 criteria where the existing formula only matches 2 criteria. Can someone tell me the format to add it to match D3 as well in the section that is bold below. I have tried changing to: MATCH($G3&"-"&$F3&"-"&$D3 but it doesn't work and I can't figure out why. It returns #N/A



=INDEX('[NEW FORM.xlsm]DATA'!$B$4:$ABV$85, MATCH($C3, '[NEW FORM.xlsm]DATA'!$A$4:$A$85, 0), MATCH($G3&"-"&$F3, INDEX('[NEW FORM.xlsm]DATA'!$B$1:$ABV$1&"-"&'[NEW FORM.xlsm]DATA'!$B$3:$ABV$3, 0), 0))
