Formula for changing Year.

Pls.help.I prepared an excel sheet for monthly plan of teaching student in class.I made 2 sheets,'monthlyplan' and 'monthlyplanentry'. I entered the data in monthlyplanentry sheet.When I change month name(G4),the data displays for each month correctly,but when I change Year from 2018 to 2019(F4),there is no change in data.I used lookup formula for data.
Pls.help which formula to be used?
