Macro to Insert Rows (with line numbers) using Command Button

HELP!!!

I am needing a Macro to insert 12 lines at a time with the same formatting and consecutive line numbers upon clicking the "Add Lines" Command Button.
Also, I only want the Macro to run if the current lines in Column B are all filled.
(Also need it to move the command button to the bottom line if possible.)

Please advise. Thank you!

Ctrl+S is used for Save. Ctrl+5 is used for Strikethrough. Why Ctrl+5? When you use hashmarks to count |||| is 4, strike through to mean 5.
