vlookup question or index

Hey guys i am trying to write a formula that will will check to see if column D is greater then 12 then it will check to see if column A matches column B then pull the information from column C into column E.
=INDEX(Sheet1!A:A,EARLYBIRDS!A14,EARLYBIRDS!H:H>12,Sheet1!C:C)
